A year since the fall of the corrupt Director Hiruma, Totei University Hospital has a new director, its first-ever female, who has started to clean up the system with her "patients first" policy. The hospital's power players conspire to revive the shady hierarchy to regain their perks and absolute authority. Once again, Michiko cuts into the hospital now facing a major turning point.

Michiko,who explicitly declines at first,sees a certain vision in her mind.

Michiko needs Hiromi's skill to save his life and tries to lift her ban, but she rejects outright and says, "I won't do surgery."
